Patient Care Network (PCNOK): What is it?
The Patient Care Network (PCNOK) is a network of primary care clinics across the state. Its goal is to help connect patients with their doctors. And it’s designed to make your healthcare experience easier.
What is a PCNOK?
A PCNOK is a network of primary care clinics across the state. The goal of this network is to make sure. Patients have access to all their PCNOK providers, regardless of where they live.
PCNoks are located in Duluth, Superior, and Thunder Bay. These three cities are home to four out of five people living in northern Ontario.
The network also aims to make primary care more accessible for patients in northern Ontario. They do this by increasing the number of doctors available and making sure that these doctors are able to provide care at all times. This is especially important in rural areas, where access to healthcare is a problem. PCNoks have clinics located throughout the north that are open on weekends and evenings so people can get the treatment they need without having to wait until Monday morning.
For Patients and Families
PCNOK is a network of primary care clinics across the state. We are one of the largest and most comprehensive health systems in New Mexico, providing access to more than 300 primary care providers.
PCNOK serves more than 1 million patients each year through our network of more than 200 healthcare facilities throughout the state, including family physicians, specialists and mental health professionals who have provided services since 1879 when this practice began as “Salazar’s Sanitarium” at 1600 Central Avenue in Las Cruces (now known as Los Robles).
As part of this network, we offer convenient access to your favorite doctor or specialist by scheduling an appointment online or over the phone—no need for lengthy waits! You can also access our network via mobile devices such as smartphones or tablets so you don’t have to worry about missing an available time slot if there is something urgent needing attention right away.
For Health Care Providers
PCNOK is a free service for health care providers, who can use it to find and refer patients to care. The network tracks patient data and helps ensure that your patients get the right treatment at the right time.
The PCNOK is designed so that it’s easy for providers to share information about their patients—from their medical histories and diagnoses to insurance information, prescription drugs they take regularly (and how much), any allergies they may have…the list goes on!
The PCNOK is used by health care providers to track patients over time. This means that if a patient sees their primary doctor, an orthopedic specialist and then a cardiologist, all of those providers can access their records in one easy-to-use system. The PCNOK also helps doctors determine when it’s time to refer a patient out for specialized care.
The PCNOK : All the information you need to know.
The Patient Care Network is a healthcare system that offers high-quality care to people who are low income, uninsured, or underinsured. PCNOK’s services include medical care and social services.
PCNOK helps patients get the information they need to make well-informed decisions about their healthcare. For example:
- PCNOK connects patients with doctors and other professionals who can help them understand their medical problems in greater detail than might be possible at a single office visit;
- PCNOK provides support groups for those facing difficult life circumstances (such as losing a job);
- PCNOK partners with community organizations such as food pantries so they can refer patients who qualify for assistance from these programs directly into their offices without having any paperwork involved;
